Your notes. Your EMR.
Connected in seconds.

Synapse transfers SOAP notes from Notation directly into your EMR — no copy-paste, no cloud exposure, no double-entry.

How Synapse Works

Three steps. One click. Your notes are where they need to be.

Complete Your Note in Notation

Finish your SOAP note as usual. Notation captures everything during the session using AI transcription.

Click Transfer in Synapse

Open your EMR, click the Synapse icon. It reads your note, maps every field to the right spot in your EMR, and shows you a preview.

Confirm and Done

Review the mapped fields, confirm, and Synapse writes them into your EMR. One click. No re-typing.

Privacy by Architecture

Patient data never leaves the clinician's computer. Here's exactly how.

Memory-Only Processing

Patient data lives in RAM during transfer, then is discarded. Nothing is written to disk, logged, or persisted.

HMAC-Signed Messaging

Every message between the Chrome extension and local daemon is HMAC-signed. Tampering is detected and rejected.

No PHI to AI

When AI assists with field mapping, it receives only structural metadata — hashed selectors and canonical field IDs. Never clinical content.

Full Transparency

Every transfer is traceable. You see exactly what was read, what was mapped, and what was written — field by field.

For Physical Therapists

Spend your time on patient care, not data entry.

Save 15+ Minutes Per Patient

No more copying SOAP fields one at a time. Synapse transfers your entire note in a single click.

Zero Transcription Errors

No re-typing means no typos, no missed fields, no compliance gaps. What you wrote is what the EMR gets.

Works With Your EMR

Kinnser, WellSky, Jane App, and more. Synapse learns your EMR's form layout and maps fields automatically.

You Stay In Control

Every transfer requires your confirmation. Synapse shows you exactly what it will write before it writes anything.

For EMR & IT Teams

Synapse integrates with your EMR without APIs, plugins, or server-side changes.

No Server-Side Footprint

Synapse runs entirely on the clinician's workstation. It interacts with your EMR through the browser — no API keys, no webhooks, no integrations to maintain.

Read-Only Until Confirmed

Synapse reads DOM elements to identify form fields. It only writes values after the clinician explicitly confirms each transfer. No background writes.

Deterministic Mapping

Field mapping follows a strict cascade: deterministic rules first, then learned patterns, then AI-assisted suggestions. AI never overrides deterministic rules.

Audit Trail

Every transfer produces a trace showing exactly what was read, mapped, and written. Traces are available for compliance review and never contain raw PHI.

Signed Binaries

Windows installers are code-signed via Azure Trusted Signing. The Chrome extension is distributed through the Chrome Web Store with a fixed, deterministic extension ID.

No Data Collection

Synapse collects zero telemetry by default. The only network calls are authentication (Keycloak) and optional structural metadata for mapping improvement — never PHI.

Privacy Policy

Synapse Extension & Companion App — Effective March 27, 2026

1. Overview

Synapse is a Chrome extension and local companion application developed by Fownd Inc. that transfers clinical documentation (SOAP notes) from the Notation platform into Electronic Medical Record (EMR) systems. This policy supplements the Fownd Inc. General Privacy Policy. Where this policy addresses topics specific to Synapse, it takes precedence for the Software.

2. What We Collect

DataPurposeWhere Stored
Fownd account emailAuthentication via Keycloak SSOLocal keychain / credential store
Extension preferencesTransfer settings, UI stateChrome storage.local (on-device)
Structural mapping metadataImproving field mapping accuracyOptional cloud sync (no PHI)

3. What We Don't Collect

  • Patient names, MRNs, dates of birth, or any Protected Health Information (PHI)
  • Clinical note content (SOAP text, assessments, treatment plans)
  • EMR credentials or session tokens
  • Browsing history, cookies, or activity on non-EMR sites
  • Screenshots, screen recordings, or DOM snapshots containing patient data

4. Local Processing

All clinical data processing happens on your local machine via the Synapse companion application (daemon). The daemon:

  • Holds PHI in memory only during active transfers
  • Discards all clinical data when the transfer completes or the application closes
  • Never writes PHI to disk, logs, databases, or temporary files
  • Communicates with the Chrome extension over a local-only channel secured with HMAC signatures

5. Extension Permissions Explained

PermissionWhy It's Needed
sidePanelProvides the main Synapse user interface as a Chrome side panel
activeTabReads the current EMR page to identify form fields for mapping
storageSaves your local preferences (never clinical data)
nativeMessagingCommunicates with the local Synapse daemon via Chrome's native messaging protocol
clipboardWriteFallback for EMR fields that cannot be auto-filled directly
identitySingle sign-on with your Fownd account via Keycloak
https://*/*Must interact with any EMR web application (EMR list is open-ended)

6. AI & Machine Learning

Synapse uses AI to assist with mapping clinical fields to EMR form elements. The AI component:

  • Receives only structural metadata: hashed CSS selectors, canonical field identifiers, DOM element roles, and field labels
  • Never receives patient data, note content, or any PHI
  • Is used as a fallback — deterministic rules always take priority over AI suggestions
  • Cannot override or replace clinician-confirmed mappings

7. Data Sharing

Fownd Inc. does not sell, rent, or share personal data or clinical information with third parties. The only external communications Synapse makes are:

  • Authentication: Keycloak identity provider (auth.fownd.care)
  • Mapping metadata (optional): Structural field mapping data sent to inference.fownd.co — contains no PHI

8. Data Retention

  • Clinical data: Zero retention. Discarded from memory after each transfer.
  • Account data: Retained while your Fownd account is active. Request deletion at privacy@fownd.care.
  • Mapping metadata: Retained for service improvement. Contains no PHI.

9. Your Rights

You have the right to:

  • Request access to any data associated with your account
  • Request deletion of your account and associated data
  • Opt out of structural mapping metadata collection
  • Withdraw consent for data processing at any time

To exercise any of these rights, contact privacy@fownd.care.
